Case Definition

Clinical case definition

  • An illness of the upper-respiratory tract characterized by laryngitis or pharyngitis or tonsillitis and an adherent membrane of the tonsils, pharynx and/or nose.

Laboratory criteria for diagnosis

  • Isolation of Corynebacterium diphtheriae from a clinical specimen

Case Classification

  • Suspected: A clinically compatible case that is not laboratory confirmed and is not epidemiologically linked to a laboratory-confirmed case.
  • Confirmed : A clinically compatible case that is either laboratory confirmed or epidemiologically linked to a laboratory-confirmed case.

Types Of Surveillance

Mandatory National Notifcation of Infectious Diseases under the Infectious Disease Prevention and Control Act 1988.

When to notify

  • Any suspected diphtheria case should be notified.

How to notify

  • A diphtheria case should be notified by phone to the nearest District Health Office within 24 hours of diagnosis. It then followed by submission of the notification form.

Outbreak situations

  • Intensive surveillance requires to be maintained during outbreaks in view of high infectivity, greater transmission risk and increased mortality.
